How to take Rybelsus: the empty-stomach rules explained
Rybelsus only works if enough of it is absorbed, and that depends on how you take it. These are the label rules in the US, UK and EU, and the reasons behind them.
Take one Rybelsus tablet a day on an empty stomach, as soon as you wake up. Swallow it whole with a sip of water, no more than 120 ml (about 4 oz, or half a glass), then wait at least 30 minutes before you eat, drink anything else or take any other tablets.12 The UK and EU labels add that the dose should follow a fast of at least 8 hours.23 The rules matter because only a tiny fraction of each dose is absorbed even when it is taken correctly, and food and drink reduce that further.1
- Wake One tablet, empty stomach Swallow it whole with a sip of plain water: no more than 120 ml (half a glass). UK and EU labels say after at least 8 hours without food.
- +30 min Wait at least 30 minutes No food, no other drinks, no other oral medicines while it is absorbed.
- Then Breakfast and other tablets Eat, drink and take your other medicines as normal.
What the US label says
The US prescribing information covers both Rybelsus and Ozempic tablets, the name the newer tablets have been sold under in the US since 2026 (see the Ozempic pill). It gives these instructions:1
- Take one tablet once daily on an empty stomach in the morning.
- Take it with water only, up to 4 ounces (about 120 ml). No other liquids.
- Swallow it whole. Do not split, crush, chew or dissolve it.
- Wait at least 30 minutes before eating, drinking or taking any other oral medicines.
- Do not take more than one tablet a day.
Unlike the UK and EU texts, the US label does not put a number of hours on the fast before the dose. It says "on an empty stomach in the morning".1
What the UK and EU labels say
In the UK the official prescribing text is the Summary of Product Characteristics (SmPC) approved by the MHRA; in the EU it is the SmPC in the EMA product information. Both use the same wording:23
- Take it on an empty stomach "after a recommended fasting period of at least 8 hours".
- Swallow it whole with a sip of water, up to half a glass (120 ml). Do not split, crush or chew it.
- Wait at least 30 minutes before eating, drinking or taking other oral medicines. Waiting less than 30 minutes decreases absorption.
The NHS puts it simply: take the tablet as soon as you wake up, having eaten nothing for at least 8 hours, with half a glass of water (up to 120 ml).4 In other words, the NHS ties the 8-hour fast to the overnight gap before you get up.
Why the rules matter: how the tablet is absorbed
Semaglutide is a peptide (a small protein) that stomach enzymes would normally break down. Each tablet contains an absorption enhancer, salcaprozate sodium (SNAC), to get it through.15 Research by Novo Nordisk scientists showed that semaglutide from the tablet is absorbed in the stomach itself, close to the tablet surface. SNAC briefly buffers the stomach around the tablet, protecting semaglutide from enzymes, and helps it cross the stomach lining.5
Even then, very little gets through. The US label puts absolute bioavailability, the share of the dose that reaches the bloodstream, at about 0.4% to 1% for the 3, 7 and 14 mg tablets and 1% to 2% for the 1.5, 4 and 9 mg tablets.1 Anything that dilutes the tablet or competes with it in the stomach lowers that small share further:
- Water volume. In studies, taking the tablet with 50 ml of water gave higher semaglutide levels than taking it with 240 ml.1
- Waiting time. A longer fast after the dose gave higher levels. The labels set 30 minutes as the minimum, and the UK and EU texts say waiting less than 30 minutes decreases absorption.12
- Other tablets. In an EU study, taking it together with five other tablets reduced semaglutide exposure by 34%.3
Absorption also varies a lot from person to person. The UK SmPC notes that 2% to 4% of patients have no exposure at all, and asks prescribers to bear this in mind if the response is lower than expected.2 If your blood sugar results are not what you and your prescriber expected, going over how you take the tablet with them is a sensible first step.
Can you drink coffee after taking Rybelsus?
Not straight away. The US label says to take the tablet with water only and to wait at least 30 minutes before drinking anything else, and the UK and EU labels also rule out drinking for at least 30 minutes.12 That covers coffee, tea, juice and milk. Once the 30 minutes have passed, the labels place no further limit on what you drink.
A routine that fits the label is to take the tablet on waking and make coffee or breakfast once 30 minutes have passed. The labels do not ask you to wait longer than that, although studies found that a longer gap after the dose gave higher semaglutide levels.1 If you are unsure what gap suits your routine, ask your pharmacist.
Other medicines, including levothyroxine
Other oral medicines have to wait until at least 30 minutes after the Rybelsus dose.12 Semaglutide also slows stomach emptying, which can change how other tablets are absorbed. For that reason the US label advises extra monitoring for oral medicines that have a narrow therapeutic index (where a small change in blood level matters) or that need clinical monitoring.1
Levothyroxine, a thyroid hormone tablet, needs particular care because it is also usually taken on waking. Total thyroxine exposure rose by 33% when levothyroxine was taken with semaglutide tablets, and the labels advise considering thyroid monitoring.12 Ask your prescriber or pharmacist how to time the two. What to do if you miss a dose
If you miss a dose, skip it and take the next tablet the following day as usual. Do not take two tablets to make up for it.124 The US label, the UK SmPC and the NHS all give the same advice.
The labels do not describe taking a missed tablet later in the day, and food reduces how much of a tablet is absorbed.1 If you find you miss doses often, talk to your pharmacist about ways to fit the tablet into your morning.
Taking more than one tablet a day is not allowed under any label, and that includes taking two lower-strength tablets to reach a higher dose. The MHRA has warned that adding tablets in this way causes overdosing.16 The Rybelsus dosage guide explains why this became a particular risk when the newer strengths arrived.
How to store Rybelsus
- US: store at 20 to 25°C (68 to 77°F); brief excursions between 15 and 30°C are allowed. Keep the tablets in the original bottle, closed, in a dry place, to protect them from moisture.1 The US Medication Guide says not to store them in any other container, and Novo Nordisk's US patient site says not to use pill organisers.17
- UK and EU: store in the original blister pack to protect the tablets from light and moisture. There is no special temperature requirement.23
In practice, that means taking a tablet out of the bottle or pressing it out of the blister only when you are about to swallow it.12
Practical tips that stay within the label
- Make it the first thing you do. The US label says to take it in the morning and the NHS says as soon as you wake up, so linking the tablet to getting up helps keep your stomach empty.14
- Keep the water small. The limit is 120 ml (4 oz), and in studies a smaller volume gave higher semaglutide levels than a larger one.1
- Time the 30 minutes. The wait applies to breakfast, coffee and your other morning tablets alike.12
- Take other morning medicines after the wait, and ask your pharmacist about any that have timing rules of their own, such as levothyroxine.1
- Use the pack, not a pill box. The tablets stay in their bottle or blister until you take one.17
- Raise problems rather than improvising. The licence and the absorption evidence both rest on these rules. If they do not fit your day, that is a conversation to have with your prescriber or pharmacist.
