Rybelsus in South Africa

What is sold here

SAHPRA registered Rybelsus 3 mg, 7 mg and 14 mg (Novo Nordisk (Pty) Ltd, registration numbers 56/21.13/0337, 0338 and 0339) on 3 September 2024. Being registered is not the same as being on sale, though: we could not find a Single Exit Price for Rybelsus in the national medicine price database (as mirrored by medicineprices.org.za, last updated 26 May 2026), and the retail pharmacy websites we checked do not list it. If your doctor prescribes it, ask your pharmacist whether it can be obtained.

What it is licensed for

Type 2 diabetes in adults, alongside diet and exercise. It is not registered for weight loss.

Prescription rules

Schedule 4 medicine: prescription-only, dispensed by a registered pharmacy (or a doctor with a dispensing licence).

How to get a prescription

Rybelsus is registered in South Africa for type 2 diabetes but we could not confirm that it is on sale. If you think it would suit you, see your GP or a specialist physician or endocrinologist, in person or by telehealth with an HPCSA-registered doctor. If Rybelsus is not obtainable, the doctor can discuss the alternatives that are on sale, such as Ozempic (injectable semaglutide) or Mounjaro (tirzepatide).

With a prescription you can use any SAPC-registered pharmacy, including retail chains and courier pharmacies that deliver chronic medicines nationwide. If you are on a medical scheme, apply for chronic medicine benefits for diabetes first: the scheme will tell you which medicines it covers, whether a co-payment applies and whether you must use a designated service provider pharmacy.

If you pay yourself, the price is the Single Exit Price plus a regulated dispensing fee, so it should be similar at every pharmacy. Oral GLP-1 medicines for weight loss, such as the Wegovy pill and Foundayo, were not registered in South Africa as of September 2026.

Cost and cover

Medical schemes (Prescribed Minimum Benefits) and the public sector

Diabetes mellitus types 1 and 2 is one of the 26 Chronic Disease List conditions that every medical scheme must cover as a Prescribed Minimum Benefit (PMB). PMB cover only obliges schemes to fund the treatment in the published PMB algorithms and their own formularies, though, so whether a newer GLP-1 medicine such as Rybelsus (or Ozempic) is paid for depends on your scheme and option, and it may need a motivation from your doctor, a co-payment or use of a designated service provider pharmacy. Weight loss is not a PMB condition, so schemes generally do not have to pay for GLP-1 medicines prescribed only for weight management. In public-sector clinics and hospitals, type 2 diabetes is treated with older medicines such as metformin, sulphonylureas and insulin; GLP-1 medicines are not part of standard state-sector treatment.

Paying yourself

We could not find a Single Exit Price for Rybelsus in South Africa, so we cannot give a reliable price. For comparison, the SEP for Ozempic is R1,826.78 or R3,352.03 depending on the pack, and for Wegovy between R2,291.63 (0.25 mg) and R4,272.14 (2.4 mg) per pack, in each case plus the pharmacy's regulated dispensing fee.

Checking a pharmacy is genuine

Every pharmacy, including an online or courier pharmacy, must be recorded with the SAPC and have a responsible pharmacist. Use the SAPC's 'Search for a Pharmacist or Pharmacy' register (linked from pharmcouncil.co.za) to search by pharmacy name or pharmacy number (Y number), and check the name of the responsible pharmacist. You can check that a medicine is registered on SAHPRA's register of health products. A legitimate pharmacy will not supply semaglutide without a prescription and must charge no more than the Single Exit Price plus the regulated dispensing fee.
