Rybelsus dosage: strengths, titration and maximum dose

Rybelsus is taken as one tablet a day. Treatment usually starts on 3 mg (or 1.5 mg with the newer tablets) for 30 days, then moves up to 7 mg (or 4 mg). After at least another 30 days the prescriber can raise it to 14 mg (or 9 mg) if more blood sugar control is needed.¹² The maximum depends on where you live: 14 mg Rybelsus or 9 mg Ozempic tablets in the US, 9 mg in the UK and 50 mg in the EU.¹²³ The two strength sets are equivalent, and every label allows only one tablet a day.¹⁴

Rybelsus strengths by region

There are two formulations of the tablet. The original oval tablets come in 3, 7 and 14 mg. A newer round tablet, from which more semaglutide is absorbed, comes in 1.5, 4 and 9 mg.¹ Which ones are sold depends on the region:

• US: Rybelsus 3, 7 and 14 mg (oval) and Ozempic tablets 1.5, 4 and 9 mg (round). The round tablets were renamed in 2026; see the Ozempic pill.¹
• EU: Rybelsus 1.5, 4 and 9 mg (round), 3, 7 and 14 mg (oval), and 25 and 50 mg (smaller oval tablets). The European Commission added the round tablets on 1 August 2024 and the 25 and 50 mg tablets on 19 September 2024.³⁵
• UK: only 1.5, 4 and 9 mg are current, and the 3, 7 and 14 mg tablets are listed as discontinued. The newer tablets launched in the UK in September 2025, and the MHRA expected old stock to run out around 31 January 2026. There is no 25 mg or 50 mg Rybelsus in the UK.⁴²

Every strength is white to light yellow, with the strength stamped on one side and "novo" on the other.¹² For the names and strengths sold in a particular country, choose your country.

Is Rybelsus 1.5 mg the same as 3 mg?

They are equivalent doses from two different tablets. The newer tablets carry lower numbers because more semaglutide is absorbed from them. Absolute bioavailability (the share of the dose that reaches the blood) is about 1% to 2% for the 1.5, 4 and 9 mg tablets and about 0.4% to 1% for the 3, 7 and 14 mg tablets.¹ The US label, the EU SmPC (Summary of Product Characteristics) and the MHRA all pair the strengths like this:¹³⁴

The US label calls the two sets "not substitutable on a mg-to-mg basis", which is why any change has to use the matching strength rather than the same number.¹ If you are moving from one set to the other, your prescriber or pharmacist should confirm the equivalent strength with you.

Why it is always one tablet a day

The US label, the EU SmPC and the MHRA all say one tablet a day. Taking two tablets to reach a higher dose is not allowed.¹³⁴

The switch to new strength numbers created a specific risk. In December 2025 the MHRA warned that adding tablets to match a previous dose number would cause overdosing. It asked prescribers to switch patients systematically to the equivalent new strength and to keep them on one tablet a day.⁴ The EMA issued a similar medication-error communication on 1 August 2025.⁶

So if the number on your new pack is lower than the number you are used to, that is expected. It is not a reason to add a second tablet. If the pack does not match what you expected, check with your pharmacist before taking it.

How the dose is increased

Titration means starting on a low dose and stepping up gradually. With Rybelsus the steps are there to reduce stomach side effects such as nausea. The starting dose is, in the US label's words, "not effective for glycemic control".¹

US label:¹

• Rybelsus: 3 mg once daily for days 1 to 30; 7 mg for days 31 to 60; from day 61, stay on 7 mg or increase to 14 mg if more glucose control is needed.
• Ozempic tablets: 1.5 mg for days 1 to 30; 4 mg for days 31 to 60; from day 61, stay on 4 mg or increase to 9 mg.

UK and EU SmPC: 1.5 mg (or 3 mg) for one month, then 4 mg (or 7 mg). After at least one month the dose can rise to 9 mg (or 14 mg). In the EU only, it can then rise to 25 mg and then to 50 mg, each after at least one month on the previous dose.²³

The labels set minimum times on each step ("at least one month", "day 61 or thereafter"), not deadlines, so a prescriber can keep someone on a lower dose for longer.¹² Dose changes are the prescriber's decision. The side effects guide covers what to expect around dose increases.

What is the maximum dose of Rybelsus?

• US: 14 mg Rybelsus or 9 mg Ozempic tablets.¹ Novo Nordisk has filed for a 25 mg Ozempic tablet for type 2 diabetes and expects an FDA decision by the end of 2026.⁷
• UK: 9 mg.²
• EU: 50 mg.³

In every region the maximum is a single tablet of the highest licensed strength, taken once a day.¹³⁴

Rybelsus 25 mg and 50 mg: the PIONEER PLUS trial

The EU's higher strengths rest on PIONEER PLUS, a 52-week trial in 1,606 adults with type 2 diabetes whose average HbA1c (a blood test of average blood sugar over recent months) was 9.0% at the start. It compared 25 mg and 50 mg with 14 mg.⁸

• HbA1c fell by 1.5, 1.8 and 2.0 percentage points with 14, 25 and 50 mg. These figures use the treatment policy estimand, which includes people who stopped taking the tablets.⁸
• Weight fell by 4.4, 6.7 and 8.0 kg by the same measure, or by 4.5, 7.0 and 9.2 kg using the trial product estimand, which leaves out people who stopped treatment.⁹

Higher doses brought more stomach side effects: in the EU SmPC, nausea was reported by 27% of people at 25 mg and 50 mg, compared with 15% at the standard doses.³ These strengths are authorised in the EU only. They are not licensed in the UK, and in the US the 25 mg application was still pending in September 2026.³²⁷ We could not confirm which EU countries stock them.

Switching from Ozempic injection to tablets

The labels handle this differently, and none of them treats it as a simple swap:

• US label: only people on the 0.5 mg weekly injection are covered. One week after the last 0.5 mg injection, they can start Rybelsus 7 mg or 14 mg, or Ozempic tablets 4 mg or 9 mg, once daily.¹
• EU SmPC: 0.5 mg weekly to 4 mg or 9 mg tablets; 1 mg weekly to 9 mg or 25 mg; 2 mg weekly to 25 mg or 50 mg, starting one week after the last injection. The SmPC warns that the effect of switching "cannot easily be predicted".³
• UK SmPC: semaglutide levels after 9 mg tablets daily are comparable to 0.5 mg injected weekly, and no tablet dose equivalent to the 1 mg injection has been established.²

Do older people, or people with kidney or liver problems, need a different dose?

No dose change is needed for older people, or for mild, moderate or severe kidney or liver impairment, although experience in end-stage kidney disease and severe liver impairment is limited.¹² When Rybelsus is started alongside insulin or a sulfonylurea (a tablet that makes the pancreas release insulin), the prescriber may lower the dose of those medicines to reduce the risk of hypoglycaemia, or low blood sugar.¹²
