Rybelsus vs Ozempic: the same medicine as a daily tablet or a weekly injection

Same medicine, different route. How the daily semaglutide tablet and the weekly injection compare on dose, evidence, licences, side effects and routine.

Rybelsus and Ozempic contain the same medicine, semaglutide, made by Novo Nordisk, and both are licensed to lower blood sugar in adults with type 2 diabetes.14 Rybelsus is a tablet you swallow every morning on an empty stomach; Ozempic is an injection under the skin once a week. So is Rybelsus as effective as Ozempic? It depends on the dose. The UK product information says a 9 mg tablet a day gives exposure (the amount of semaglutide in the blood over time) comparable to a 0.5 mg injection a week, and that no tablet dose equivalent to the 1 mg injection has been established.2 The tablet (PIONEER) and injection (SUSTAIN) trials were run separately, so their results are not a head-to-head contest.

Same medicine, two ways of taking it

Semaglutide is a GLP-1 receptor agonist: it copies GLP-1, a hormone the gut releases after eating. It makes the pancreas release more insulin and less glucagon when blood sugar is raised, and slightly slows how fast the stomach empties.1 It also reduces appetite, so weight often falls, although neither product is licensed for weight loss.1114

Semaglutide is a peptide (a small protein) that stomach enzymes would normally break down. Rybelsus tablets contain an absorption enhancer, SNAC, that protects it and helps it cross the stomach lining. Even so, bioavailability (the share of a dose that reaches the blood) is only about 0.4% to 1% for the 3, 7 and 14 mg tablets and 1% to 2% for the 1.5, 4 and 9 mg tablets.1 That is why tablet doses are whole milligrams while injection doses are fractions of a milligram.

How the doses compare

Ozempic starts at 0.25 mg a week for 4 weeks, then 0.5 mg, and can rise to 1 mg and a maximum of 2 mg, with at least 4 weeks on each dose.4 Rybelsus starts at 1.5 mg (or 3 mg) a day for a month, then 4 mg (or 7 mg), and can rise to 9 mg (or 14 mg). The two tablet sets are equivalent: 1.5 mg equals 3 mg, 4 mg equals 7 mg and 9 mg equals 14 mg. The starting tablet dose does not control blood sugar on its own; it is there to reduce stomach side effects. It is always one tablet a day.12

The UK SmPC (the official product information) says a 9 mg tablet daily gives exposure comparable to a 0.5 mg injection weekly, and no tablet dose equivalent to 1 mg injected has been established.2 So under the UK licence, the top tablet dose sits at about the second step of the injection.

Tablet absorption also varies a lot between people. The UK SmPC notes that 2% to 4% of patients have no exposure at all.2

Is Rybelsus as effective as Ozempic?

Blood sugar results are reported as HbA1c, a blood test that reflects average blood sugar over recent months. These PIONEER tablet results are from the US label, over 26 weeks, counting everyone whether or not they kept taking the medicine:1

  • PIONEER 1, against placebo (a dummy tablet): HbA1c fell 1.2 points with 7 mg and 1.4 with 14 mg, vs 0.3. Weight fell 2.3 kg and 3.7 kg, vs 1.4 kg.1
  • PIONEER 4, against liraglutide 1.8 mg (another GLP-1 injection) and placebo: HbA1c fell 1.2 points vs 1.1 and 0.2, showing the tablet was no worse than liraglutide.1

In SUSTAIN FORTE (40 weeks), which compared two Ozempic doses, HbA1c fell 2.2 points with 2 mg and 1.9 with 1 mg.8 These figures use a "trial product" analysis that leaves out people who stopped treatment, which usually gives higher numbers.

Do not read these side by side as a contest. The trials differ in the people enrolled, length (26 vs 40 weeks), comparators and analysis. The closest thing to a direct comparison in the labels is the exposure statement: a 9 mg tablet is comparable to a 0.5 mg injection, and the injection can go higher.2 What that means for you is a conversation for your prescriber.

Heart and kidney evidence

SOUL tested the tablet against placebo in 9,650 adults aged 50 or over with type 2 diabetes and cardiovascular disease, chronic kidney disease or both, for an average of 47.5 months. Major cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke) occurred in 12.0% vs 13.8%, a 14% relative reduction; a kidney outcome was not significantly different.10 The FDA added a cardiovascular indication in October 2025; the EU and UK added the results to the product information without a new indication.12

For the injection, SUSTAIN-6 studied cardiovascular events and FLOW studied kidney outcomes.79 In the US, Ozempic injection is licensed to reduce major cardiovascular events in adults with type 2 diabetes and established cardiovascular disease and, since January 2025, to reduce the risk of kidney failure, sustained decline in kidney function and cardiovascular death in those with chronic kidney disease.46 In the EU and UK these results are in the product information, not separate indications.5

Side effects and safety

The US tablet label lists nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation as the most common side effects. In placebo-controlled trials nausea affected 11% on 7 mg and 20% on 14 mg, vs 6% on placebo, and 8% on 14 mg stopped because of stomach or gut effects, mostly while the dose was going up.1 Ozempic injection reports its own figures from its own trials, so the percentages are not directly comparable.

Some warnings apply to semaglutide in either form. Diabetic retinopathy (diabetes-related eye disease) complications were more frequent with semaglutide in SUSTAIN-6, especially in people who already had retinopathy.1 The EMA safety committee concluded in June 2025 that NAION, a sudden loss of vision in one eye, is a very rare side effect of semaglutide medicines.14 The tablet label also lists pancreatitis, gallbladder problems, kidney injury from dehydration, and low blood sugar with insulin or a sulfonylurea.1 Semaglutide products should not be combined with each other.15 See Rybelsus side effects for more.

Switching between the injection and the tablet

Switching is a prescriber decision, and the rules differ by region:

  • US: the tablet label covers only people on the 0.5 mg weekly injection. One week after the last injection they can start Rybelsus 7 mg or 14 mg, or Ozempic tablets 4 mg or 9 mg.1
  • EU: the SmPC maps 0.5 mg injections to 4 mg or 9 mg tablets, 1 mg to 9 mg or 25 mg, and 2 mg to 25 mg or 50 mg, starting one week after the last injection. It warns that the effect of switching "cannot easily be predicted".3
  • UK: with a 9 mg maximum and no tablet dose established as equivalent to 1 mg injected, no UK tablet dose matches the higher injection doses.2

Other countries follow their own product information; see Rybelsus by country. Do not stop one and start the other on your own; semaglutide stays in the blood for about five weeks after the last dose.1

Convenience and cost trade-offs

The tablet is taken in the morning on an empty stomach with no more than 120 ml of water, and you then wait at least 30 minutes before eating, drinking or taking other tablets. The UK and EU labels add a fast of at least 8 hours beforehand.12 Other morning medicines have to fit around that wait: levothyroxine exposure rose by 33% when taken with semaglutide tablets, so people on thyroid medicine should ask how to time the two.1 The injection is once a week with no fasting or water rules, and pens are kept in the fridge until use.411 Needles versus a daily routine is a personal trade-off. In the US, Ozempic is now also a tablet

On 30 January 2026 the FDA approved renaming the newer Rybelsus tablets (1.5, 4 and 9 mg) as Ozempic tablets, available from 4 May 2026.1213 In the US, "Ozempic" can now mean the weekly injection or the daily tablet. Rybelsus 3, 7 and 14 mg remain on the same label, which says the two tablet sets are "not substitutable on a mg-to-mg basis".1 Outside the US the tablet is still Rybelsus.13 Check which product and strength is on your prescription. Questions people ask

Is Rybelsus as effective as Ozempic?

It depends on the doses being compared. The UK product information says a 9 mg Rybelsus tablet taken daily gives semaglutide exposure comparable to a 0.5 mg Ozempic injection taken weekly, and that no tablet dose equivalent to the 1 mg injection has been established. The tablet (PIONEER) and injection (SUSTAIN) trials were run separately, with different people, lengths and comparators, so their results should not be read as a head-to-head comparison. Your prescriber can explain what the evidence means for you.

Is Rybelsus the same as Ozempic?

They contain the same active ingredient, semaglutide, from the same manufacturer, Novo Nordisk. Rybelsus is a tablet taken once a day on an empty stomach; Ozempic is an injection given once a week. The milligram doses are very different because only a small fraction of a tablet dose is absorbed. In the US, the Ozempic name now also covers the newer 1.5, 4 and 9 mg tablets.

What Rybelsus dose is equivalent to Ozempic?

The UK label says 9 mg tablets daily give exposure comparable to 0.5 mg injected weekly (9 mg matches the older 14 mg tablet). The US label lets people on the 0.5 mg injection move to Rybelsus 7 mg or 14 mg, or Ozempic tablets 4 mg or 9 mg. No UK tablet dose equivalent to the 1 mg injection has been established; the EU label maps 1 mg to 9 mg or 25 mg tablets. Only your prescriber can decide on a switch.

Can I switch from Ozempic injections to Rybelsus tablets?

The labels describe how a switch can be done, but the rules vary by region and the decision belongs to your prescriber. In the US, the label covers only people on the 0.5 mg weekly injection, who start tablets one week after their last injection. The EU label covers 0.5, 1 and 2 mg injections and warns that the effect of switching cannot easily be predicted. Do not stop or change your medicine without speaking to your prescriber.

Does Rybelsus have fewer side effects than Ozempic?

The labels do not compare the two directly. The tablet label lists nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation as the most common side effects, mostly while the dose is being increased. The injection label reports its own figures from different trials, so the percentages cannot be set against each other.

Is the Ozempic pill the same as Rybelsus?

In the US, the FDA approved renaming the newer Rybelsus 1.5, 4 and 9 mg tablets as Ozempic tablets in January 2026, and they became available in May 2026. They contain the same medicine and follow the same rules. Outside the US the tablet is still called Rybelsus. The Ozempic pill is not the Wegovy pill, which is a separate product licensed for weight management.

Can you take Rybelsus and Ozempic together?

No. Both are semaglutide, and semaglutide products should not be combined with each other or with other GLP-1 medicines. If you have been prescribed one and are offered the other, check with your prescriber or pharmacist.

Which causes more weight loss, Rybelsus or Ozempic?

Neither is licensed for weight loss; both are licensed for type 2 diabetes, and weight often falls as a secondary effect. The tablet and injection trials report weight in different trials with different lengths and comparators, so they cannot be ranked. The semaglutide products licensed for weight management are Wegovy injections and the Wegovy pill.
