Rybelsus vs metformin: how they compare, and taking them together

Metformin is the long-standing first-line tablet for type 2 diabetes; Rybelsus is a newer GLP-1 tablet that is often added to it. Here is how they differ and how the two fit into one morning routine.

Metformin and Rybelsus both treat type 2 diabetes, but they usually work as partners rather than rivals. Metformin is an older, inexpensive generic tablet taken with meals, and guidelines place it at the start of treatment: NICE recommends modified-release metformin plus an SGLT2 inhibitor as initial therapy for most adults45. Rybelsus (oral semaglutide) is a newer once-daily GLP-1 tablet that is often added to metformin rather than replacing it45. In the UK and EU, Rybelsus is licensed on its own only when metformin is unsuitable, and otherwise alongside other diabetes medicines23. You can take the two together if your prescriber recommends it: Rybelsus first thing on an empty stomach, then wait at least 30 minutes before eating or taking metformin1.

The two trial rows come from different trials with different lengths, populations and starting points, so they are not a head-to-head comparison.

Where guidelines place metformin and Rybelsus

Metformin is the older medicine, and guidelines put it at the start of treatment. NICE guideline NG28, last updated on 18 February 2026, recommends modified-release metformin together with an SGLT2 inhibitor (another class of tablet; see Rybelsus vs Jardiance) as initial therapy for most adults5. The American Diabetes Association's 2026 Standards of Care rate metformin's glucose-lowering efficacy as high7.

In NICE's guideline, GLP-1 medicines appear in the chapter on further medication. NG28 names the GLP-1 receptor agonist options as liraglutide, dulaglutide and semaglutide, and recommends injected semaglutide for people with atherosclerotic cardiovascular disease (heart or circulation disease caused by narrowed arteries). It does not address oral semaglutide separately56. The ADA rates GLP-1 receptor agonists as having high to very high glucose-lowering efficacy and lists cardiovascular benefit for both injected and oral semaglutide7.

The licences follow a similar order. In the UK and EU, Rybelsus can be used on its own only when metformin is considered inappropriate (the UK SmPC adds "due to intolerance or contraindications"), and otherwise in combination with other diabetes medicines23. The US indication does not mention metformin, and since October 2025 it also covers reducing the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes at high risk1. Metformin has no cardiovascular-outcome indication4.

Can you take Rybelsus with metformin?

Yes, if your prescriber recommends it. Rybelsus is often added to metformin rather than used instead of it45, and the UK and EU licences allow it in combination with other diabetes medicines23. PIONEER 2, one of the main Rybelsus trials, was run in 822 adults who were already taking metformin1.

The two work in different ways. Rybelsus contains semaglutide, which copies GLP-1 (glucagon-like peptide-1), a hormone the gut releases after eating. It makes the pancreas release more insulin and less glucagon when blood sugar is raised, and slightly slows stomach emptying1. It also reduces appetite, which is why weight often falls8. Metformin belongs to a separate class of medicine, the biguanides4.

In interaction studies, semaglutide tablets had no clinically relevant effect on metformin levels12. Semaglutide does slow stomach emptying, which can change how other tablets are absorbed, so the US label advises extra monitoring for oral medicines that need careful dosing or monitoring1.

Blood sugar and weight: what the trials show

Diabetes trials measure HbA1c, a blood test that reflects average blood sugar over recent months.

Metformin. In a 29-week trial described in the US metformin label, HbA1c fell by 1.4 points on metformin, while it rose by 0.4 points on placebo. Weight changed by -1.4 lb (about 0.6 kg) on metformin and -2.4 lb (about 1.1 kg) on placebo, which the label treats as weight neutral4.

Rybelsus on its own. In PIONEER 1 (26 weeks, 703 adults, compared with placebo), HbA1c fell by 1.2 points on 7 mg and 1.4 points on 14 mg, against 0.3 points on placebo. Weight fell by 2.3 kg and 3.7 kg, against 1.4 kg1.

Rybelsus added to metformin. In PIONEER 2 (26 weeks, adults already on metformin), Rybelsus 14 mg lowered HbA1c by 1.3 points, against 0.9 points with empagliflozin 25 mg, with similar weight loss (3.8 kg vs 3.7 kg)1. These are the US label's treatment-policy results, which include people who stopped treatment1.

These figures come from separate trials, so they show what each medicine did in its own trial rather than which lowers blood sugar more. Rybelsus is not licensed for weight loss; weight change is a secondary effect seen in the diabetes trials13. See Rybelsus and weight loss.

Side effects: both can upset the stomach

Stomach and bowel (gastrointestinal, or GI) side effects are common with both medicines14.

Rybelsus. The most common side effects are nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation. In placebo-controlled trials, any GI side effect was reported by 41% on 14 mg, 32% on 7 mg and 21% on placebo, and most nausea, vomiting and diarrhoea happened while the dose was being increased1. The dose steps at the start are there to reduce these effects1.

Metformin. GI side effects are common. The US label carries a boxed warning about lactic acidosis, a serious build-up of lactic acid in the blood, and long-term use can lower vitamin B12 levels47.

Kidneys. Metformin must not be used if eGFR (a measure of kidney function) is below 30, and should not be started if it is between 30 and 454. Rybelsus needs no dose change for kidney impairment, but vomiting or diarrhoea can cause dehydration and acute kidney injury1. If stomach symptoms are severe or do not settle, drink fluids and contact your prescriber or pharmacist1. See Rybelsus side effects.

Low blood sugar (hypoglycaemia) risk

Hypoglycaemia means blood sugar falling too low. Semaglutide's effect on insulin depends on blood sugar being raised, so its risk of hypoglycaemia is low when it is used without other medicines that cause it12. The risk rises when Rybelsus is combined with insulin or a sulfonylurea, and the prescriber may lower those doses12.

The UK and EU product information list hypoglycaemia as common (between 1 in 100 and 1 in 10 people) when Rybelsus is used with other oral diabetes medicines, and very common (more than 1 in 10) with insulin or a sulfonylurea23. Hypoglycaemia with insulin or a sulfonylurea can affect driving2. Ask your prescriber or pharmacist which symptoms to watch for with your particular combination.

Taking them together: the 30-minute rule

Metformin is taken with meals4. Rybelsus has to be taken before any food, drink or other tablets. A morning that follows both sets of instructions looks like this128:

  1. On waking, having eaten nothing for at least 8 hours (the UK and EU wording), take Rybelsus with a sip of plain water, up to 120 ml (4 oz). Swallow it whole.
  2. Wait at least 30 minutes: no food, no other drinks and no other tablets, including metformin.
  3. Then eat, and take metformin with the meal as prescribed.

Waiting less than 30 minutes reduces how much semaglutide is absorbed2. If you miss a Rybelsus dose, skip it and take the next one the following day1. Questions people ask

Can you take Rybelsus with metformin?

Yes, if your prescriber recommends it. Rybelsus is often added to metformin rather than replacing it, the UK and EU licences allow it with other diabetes medicines, and PIONEER 2 tested it in adults already taking metformin. Interaction studies found no clinically relevant effect of semaglutide tablets on metformin levels.

The main thing to get right is timing: Rybelsus goes first, on an empty stomach, and metformin waits at least 30 minutes.

Should I take Rybelsus and metformin at the same time?

No. Rybelsus must be taken on its own, on an empty stomach with up to 120 ml of plain water, and you then wait at least 30 minutes before eating, drinking or taking any other tablets, including metformin. Metformin is taken with meals, so one way to follow both sets of instructions is Rybelsus on waking and metformin with breakfast at least 30 minutes later. Your prescriber or pharmacist can confirm the timing for your doses.

Is Rybelsus stronger than metformin?

The published figures do not answer that directly, because they come from different trials. In PIONEER 1, Rybelsus 14 mg lowered HbA1c by 1.4 points over 26 weeks against 0.3 on placebo. In a 29-week trial described in the US metformin label, metformin lowered HbA1c by 1.4 points while it rose by 0.4 on placebo.

The American Diabetes Association rates metformin's glucose-lowering efficacy as high and GLP-1 medicines as high to very high. The two are often used together rather than as alternatives.

Can Rybelsus replace metformin?

That is a decision for your prescriber. In the UK and EU, Rybelsus is licensed on its own only when metformin is considered inappropriate, for example because of intolerance or contraindications; otherwise it is licensed in combination with other diabetes medicines. Do not stop metformin or any other diabetes medicine without speaking to your prescriber.

Does taking Rybelsus with metformin cause low blood sugar?

No. Metformin is a biguanide. Rybelsus contains semaglutide, a GLP-1 receptor agonist that copies a gut hormone. They are different classes of medicine, and Rybelsus is often prescribed alongside metformin.

Which causes more weight loss, metformin or Rybelsus?

In the trials quoted here, metformin was weight neutral, while people on Rybelsus lost a few kilograms on average over 26 weeks in the placebo-controlled PIONEER trials. Rybelsus is not licensed for weight loss, though; weight change is a secondary effect seen in the diabetes trials.
