Rybelsus for weight loss: what the licence and the trials say

No. Rybelsus is not licensed for weight loss anywhere. The FDA, the EMA and the MHRA license it only for adults with type 2 diabetes (and, in the US, to reduce cardiovascular risk in high-risk adults with type 2 diabetes), and no regulator has authorised it for weight management.¹²³ Weight loss is a secondary effect of the diabetes treatment: in the 26-week PIONEER trials, people on 7 mg or 14 mg lost on average about 2.3 to 4.4 kg, compared with 0.4 to 1.4 kg on placebo.¹ The tablets that are licensed for weight management are a different semaglutide product, the Wegovy pill, and Eli Lilly's Foundayo.⁶¹¹

Is Rybelsus approved for weight loss?

A medicine's licence lists the uses that regulators have approved, based on trials in the people who will take it. Rybelsus was developed and licensed for blood sugar control in type 2 diabetes.¹²³ Its current indications are:

• US (FDA prescribing information): blood sugar control in adults with type 2 diabetes, alongside diet and exercise; and reducing the risk of major cardiovascular events in adults with type 2 diabetes at high risk of them.¹
• EU (EMA product information) and UK (the MHRA-approved Summary of Product Characteristics, or SmPC): adults with insufficiently controlled type 2 diabetes, alongside diet and exercise, on its own or with other diabetes medicines.²³

Weight management is not on either list. Using Rybelsus purely for weight loss would be off-label, meaning outside the uses it is licensed for. The same applies to the US Ozempic pill, which is the newer Rybelsus tablet under another name.¹

How much weight did people lose in the Rybelsus trials?

The PIONEER trials enrolled adults with type 2 diabetes, and weight was one of several things measured alongside blood sugar. These are the average changes reported in the US label, over 26 weeks, using the treatment policy analysis, which includes people who stopped treatment:¹

Two longer studies add to the picture. In SOUL, a cardiovascular outcomes trial of oral semaglutide up to 14 mg in adults with type 2 diabetes, weight at week 104 was 4.22 kg lower than at the start, compared with 1.27 kg lower on placebo.² In PIONEER PLUS, a 52-week trial of the higher doses licensed only in the EU, weight fell by 6.7 kg on 25 mg and 8.0 kg on 50 mg, compared with 4.4 kg on 14 mg (treatment policy estimand).⁵

These are group averages from trials, not a prediction for any one person.

Why does Rybelsus affect weight?

Semaglutide copies GLP-1 (glucagon-like peptide-1), a hormone the gut releases after eating. Alongside its effects on insulin and blood sugar, it slightly slows how fast the stomach empties after a meal.¹ The NHS says semaglutide also reduces appetite and helps you feel full for longer, which is why weight often falls.⁴ The EU SmPC lists decreased appetite as a common side effect and weight decrease as an uncommon one.²

Why Rybelsus has no weight-loss licence

Medicines are licensed for specific uses on the strength of trials in those groups of people. Rybelsus was developed and licensed for blood sugar control in type 2 diabetes (and, in the US, cardiovascular risk reduction), and no regulator has authorised it for weight management.¹²³ Instead, Novo Nordisk ran separate obesity trials, called OASIS, of higher oral semaglutide doses in people without diabetes. The product that came out of them is the Wegovy pill.⁷⁶

So although the Wegovy pill contains the same active ingredient, it is a different product, with a 25 mg maintenance dose and its own licence. The US labels say it is not interchangeable with Rybelsus or the Ozempic pill, even where strengths share the same numbers, and semaglutide products should not be combined with each other or with other GLP-1 medicines.⁶¹

The Wegovy pill: oral semaglutide licensed for weight management

The Wegovy pill is a once-daily semaglutide tablet with the same empty-stomach routine as Rybelsus. The dose steps up through 1.5 mg, 4 mg and 9 mg, 30 days each, to a 25 mg maintenance dose from day 91.⁶ It was approved by the FDA on 22 December 2025, by the MHRA on 11 June 2026 and by the European Commission on 14 July 2026.⁸⁹¹⁰

Its main trial, OASIS 4, followed 307 adults with obesity, or overweight with a related health condition, who did not have diabetes, for 64 weeks:⁶⁷

• average weight change was -13.6% with 25 mg vs -2.4% with placebo in the FDA label's analysis, or -13.6% vs -2.2% in the published trial;⁶⁷
• 76.3% lost at least 5% of their body weight (vs 31.3% on placebo), and 27.9% lost at least 20% (vs 3.1%);⁶
• gastrointestinal (stomach and bowel) adverse events affected 74.0% of people on the pill vs 42.2% on placebo.⁷

One limit matters to anyone reading this because they have type 2 diabetes: the US label notes that the Wegovy tablets have not been studied for weight loss in people with type 2 diabetes, in whom semaglutide levels were lower.⁶ For a side-by-side comparison, see Rybelsus vs the Wegovy pill and the Wegovy pill profile.

Foundayo: a weight-management pill without fasting rules

Foundayo (orforglipron), from Eli Lilly, is a different kind of GLP-1 medicine: a small non-peptide molecule that needs no absorption enhancer and, under its US label, can be taken with or without food.¹¹ The FDA approved it on 1 April 2026 for weight management only.¹² The MHRA authorised it on 10 August 2026 for both weight management and type 2 diabetes.¹³

• ATTAIN-1 (72 weeks, 3,127 adults with obesity, without diabetes): weight change of -7.5%, -8.4% and -11.2% at the 6, 12 and 36 mg trial doses, vs -2.1% with placebo. The 36 mg trial dose corresponds to the 17.2 mg tablet that is sold.¹⁴¹¹
• ATTAIN-2 (72 weeks, 1,613 adults with obesity and type 2 diabetes): -5.1%, -7.0% and -9.6%, vs -2.5% with placebo.¹⁵

In ATTAIN-1, 5.3% to 10.3% of people stopped because of adverse events, compared with 2.7% on placebo.¹⁴ See Rybelsus vs Foundayo and the Foundayo profile for more.

Comparing the numbers fairly

It is tempting to line these results up, but the trials were not designed to be compared:

• Different people. PIONEER enrolled adults with type 2 diabetes. OASIS 4 and ATTAIN-1 enrolled adults with obesity or overweight who did not have diabetes. ATTAIN-2 is the one weight-management trial here in people with type 2 diabetes, and its results were smaller than those of ATTAIN-1.¹⁷¹⁴¹⁵
• Different lengths. 26 weeks for the PIONEER label results, 64 weeks for OASIS 4 and 72 weeks for ATTAIN-1 and ATTAIN-2.¹⁷¹⁴¹⁵
• Different measures. The PIONEER results are in kilograms; OASIS and ATTAIN report the change as a percentage of body weight.
• No head-to-head weight trial. There is no direct weight-loss trial of the Wegovy pill against Foundayo, so comparisons between OASIS 4 and ATTAIN-1 are indicative only.⁷¹⁴ The only direct comparison of two GLP-1 pills, ACHIEVE-3, tested Foundayo against oral semaglutide in 1,698 adults with type 2 diabetes over 52 weeks, and its main results were for HbA1c (a blood test of average blood sugar over recent months), not weight.¹⁶

Off-label prescribing is a prescriber's decision

Using a medicine outside its licence, known as off-label prescribing, is a clinical decision that only a prescriber can make, weighing your health against the licensed options.
