Rybelsus vs the Wegovy pill: two semaglutide tablets with different licences

Rybelsus and the Wegovy pill are both once-daily semaglutide tablets from Novo Nordisk, and you take them the same way, but they are separate products with different licences. Rybelsus is licensed to control blood sugar in adults with type 2 diabetes and is not licensed for weight loss.¹³ The Wegovy pill is licensed for weight management, with a 25 mg maintenance dose, well above Rybelsus's top dose of 9 mg in the UK or 14 mg in the US.²³¹ The evidence comes from different trials: PIONEER for Rybelsus, in people with diabetes, and OASIS for the Wegovy pill, in people without diabetes. So the results cannot be compared directly, and the two are not interchangeable, even where strengths share a number.²¹

Same molecule, two licences

Semaglutide is a GLP-1 receptor agonist. It copies GLP-1, a hormone the gut releases after eating, which raises insulin when blood sugar is high and slightly slows stomach emptying. It also reduces appetite and helps people feel full for longer.¹⁶ What differs is what regulators have authorised each tablet to treat, based on the trials run in each group of people:

• Rybelsus: adults with type 2 diabetes, alongside diet and exercise. In the US the label also covers reducing the risk of major cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke) in adults with type 2 diabetes at high risk.¹ In the EU and UK it is licensed for type 2 diabetes only.⁴³
• Wegovy pill: weight management in the US, UK and EU. In the US the tablets also carry an indication to reduce the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight.²⁶⁷

No regulator lists weight management as a Rybelsus indication. Weight loss in its diabetes trials was a secondary effect, and using it purely for weight loss would be off-label, meaning outside its licence.¹⁴³ Novo Nordisk ran separate obesity trials, called OASIS, and the product that came from them is the Wegovy pill.¹¹²

Doses: overlapping numbers, different products

The Wegovy pill starts at 1.5 mg once a day, then steps up to 4 mg and 9 mg, spending 30 days on each, and reaches the 25 mg maintenance dose from day 91.² Rybelsus shares some of those numbers. In the UK it comes as 1.5, 4 and 9 mg, and 9 mg is the maximum.³ In the US the older Rybelsus strengths are 3, 7 and 14 mg, while 1.5, 4 and 9 mg are now sold as Ozempic tablets.¹ The EU also authorises Rybelsus 25 mg and 50 mg for type 2 diabetes.⁴

Shared numbers do not make them interchangeable. The Wegovy pill is a separate product from Rybelsus and Ozempic tablets, and semaglutide products should not be combined with each other or with other GLP-1 medicines.²¹ The Rybelsus labels allow only one tablet a day, so taking extra tablets to reach a higher dose is not allowed.¹⁴ Any move from one product to the other has to come from your prescriber. What the trials show: OASIS vs PIONEER

Wegovy pill. In OASIS 4, 307 adults with obesity, or overweight with a related health condition, and without diabetes took 25 mg or placebo (a dummy tablet) for 64 weeks. Average weight fell by 13.6% vs 2.4% with placebo in the FDA label analysis; the NEJM paper reports 13.6% vs 2.2%.²¹¹ According to the FDA label, 76.3% lost at least 5% of their body weight (vs 31.3% on placebo), 59.8% lost at least 10% (vs 14.4%) and 27.9% lost at least 20% (vs 3.1%).² An earlier trial, OASIS 1, tested a 50 mg dose, which is not the dose approved for weight management.¹²²

Rybelsus. The PIONEER trials enrolled people with type 2 diabetes and measured HbA1c, a blood test that reflects average blood sugar over recent months. In PIONEER 1 (26 weeks, against placebo), HbA1c fell 1.2 points with 7 mg and 1.4 with 14 mg, vs 0.3 with placebo.¹ Across the placebo-controlled PIONEER trials, average weight loss at 7 mg and 14 mg over 26 weeks ranged from about 2.3 to 4.4 kg, vs 0.4 to 1.4 kg with placebo.¹ In PIONEER PLUS (52 weeks, 1,606 adults with type 2 diabetes), the EU-only 25 mg and 50 mg doses were compared with 14 mg: HbA1c fell 1.8 and 2.0 points vs 1.5, and weight fell 6.7 kg and 8.0 kg vs 4.4 kg.¹³¹⁴

These results are not a head-to-head comparison. The trials differ in who took part (with or without diabetes), length (26 or 52 weeks vs 64 weeks) and how weight is reported (kilograms in PIONEER, percentage of body weight in OASIS). The US Wegovy label also notes that the tablets have not been studied for weight loss in people with type 2 diabetes, in whom semaglutide levels were lower.² So OASIS results cannot simply be applied to someone with type 2 diabetes. Side effects: what the trials reported

The US Rybelsus label lists nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation as the most common side effects. In placebo-controlled trials nausea affected 11% on 7 mg and 20% on 14 mg, vs 6% on placebo, mostly while the dose was going up.¹ In OASIS 4, gastrointestinal adverse events were reported by 74.0% of people on the Wegovy pill vs 42.2% on placebo.¹¹ At the EU-only 25 mg and 50 mg Rybelsus doses, nausea was reported by 27%.⁴ These figures come from different trials, doses and ways of counting, so they are not like for like.

Serious warnings in the Rybelsus label include pancreatitis, gallbladder problems, kidney injury from dehydration, and diabetic retinopathy complications in people with diabetes.¹ The EMA safety committee concluded in June 2025 that NAION, a sudden loss of vision in one eye, is a very rare side effect of semaglutide medicines.¹⁵ See Rybelsus side effects for more.

Taking them: the same morning routine

In the US, both tablets are taken on an empty stomach in the morning with up to 4 ounces (about 120 ml) of water, swallowed whole, followed by a wait of at least 30 minutes before food, drink or other oral medicines.¹² The UK and EU labels add a fast of at least 8 hours before the dose.³⁵ Food, drink and extra water reduce how much semaglutide is absorbed, which is why the rules matter.¹ If you want the routine step by step, see how to take Rybelsus.

Where each is available

The Wegovy pill is much newer. The FDA approved it on 22 December 2025 and it launched in the US on 5 January 2026.⁸⁹ The MHRA approved it on 11 June 2026, and the European Commission authorised it on 14 July 2026 as an extension of the Wegovy licence.⁶¹⁰ On 15 July 2026 Novo Nordisk said it was available in the US, UK and UAE, with more launches planned for the second half of 2026.¹⁰ At approval the MHRA said it was not available on the NHS; any NHS use depends on NICE.⁶ In the US, the 1.5, 4 and 9 mg Rybelsus tablets are now called Ozempic tablets (see the Ozempic pill).¹
