Rybelsus vs Foundayo: oral semaglutide compared with orforglipron

Rybelsus and Foundayo are both once-daily GLP-1 tablets, but they are different medicines from different companies. Rybelsus is semaglutide from Novo Nordisk, a peptide that needs strict fasting and water rules to be absorbed. Foundayo is orforglipron from Eli Lilly, a small non-peptide molecule that can be taken with or without food.¹⁴ Their licences differ too: Rybelsus is licensed for type 2 diabetes, while Foundayo is licensed for weight management in the US and for both weight management and type 2 diabetes in the UK.¹⁶¹⁰ In the only head-to-head trial, ACHIEVE-3, orforglipron lowered blood sugar more than oral semaglutide in people with type 2 diabetes, with more stomach side effects and more people stopping treatment.¹²

Peptide vs small molecule: why it matters

Both medicines are GLP-1 receptor agonists. They act like GLP-1, a hormone the gut releases after eating, which helps the pancreas release insulin when blood sugar is high and slightly slows stomach emptying.¹ The difference is the chemistry.

Semaglutide is a peptide, a small protein that stomach enzymes would normally break down. Rybelsus tablets contain an absorption enhancer, SNAC (salcaprozate sodium), which protects semaglutide close to the tablet surface and briefly helps it cross the stomach lining.¹¹⁶ Only a small share is absorbed (about 1% to 2% for the 1.5, 4 and 9 mg tablets), and food, drink and extra water reduce it further.¹

Orforglipron is a small-molecule, non-peptide GLP-1 receptor agonist, so it needs no absorption enhancer.⁴⁵ It also comes without the fasting and water rules, as described below.

Rybelsus vs Foundayo at a glance

Taking them: fasting rules vs any time of day

Rybelsus is taken once a day in the morning on an empty stomach with no more than 120 ml (4 oz) of water. You swallow it whole and wait at least 30 minutes before eating, drinking or taking other tablets. The UK and EU labels add a fast of at least 8 hours before the dose.¹² Other morning medicines, such as levothyroxine for an underactive thyroid, have to fit around that wait.¹

Foundayo is one tablet a day, with or without food, according to the US label; the UK SmPC says it can be taken at any time of day without food or water restrictions.⁴⁵ It starts at 0.8 mg and steps up to 2.5 mg and 5.5 mg, then can rise to 9, 14.5 or 17.2 mg, with at least 30 days on each step.⁴⁵ Rybelsus steps from 1.5 mg to 4 mg and, if more blood sugar control is needed, 9 mg (or 3, 7 and 14 mg for the older strengths).²¹

For some people the Rybelsus routine fits easily into the start of the day; for others, such as those who take several morning tablets, a medicine without food rules may be simpler. Licences by region

• US: Rybelsus, and the same tablets sold as Ozempic tablets, are licensed for blood sugar control in adults with type 2 diabetes and to reduce the risk of major cardiovascular events in those at high risk.¹ The FDA approved Foundayo on 1 April 2026 for weight management only.⁶⁷ Lilly submitted a type 2 diabetes application in 2026, which was still pending in September 2026.⁸⁹
• UK: Rybelsus is licensed for type 2 diabetes.² The MHRA authorised Foundayo on 10 August 2026 for both weight management and type 2 diabetes, the first authorisation in Europe.¹⁰
• EU: Rybelsus is licensed for type 2 diabetes, with doses up to 50 mg.³ Foundayo is under review by the EMA.¹¹

Rybelsus is not licensed for weight loss by any of these regulators.¹²³ So in the US the two currently share no licensed use: Rybelsus for diabetes, Foundayo for weight management. In the UK they overlap for type 2 diabetes. For how the US naming works, see the Ozempic pill.

Head to head in type 2 diabetes: ACHIEVE-3

ACHIEVE-3 is the only trial that compares the two tablets directly. It followed 1,698 adults with type 2 diabetes who were already taking metformin, for 52 weeks. It was open label, meaning participants and doctors knew which medicine each person was taking.¹²

• HbA1c (a blood test that reflects average blood sugar over recent months) fell by 1.71 points with orforglipron 12 mg and 1.91 with 36 mg, vs 1.23 with oral semaglutide 7 mg and 1.47 with 14 mg.¹²
• Both orforglipron doses were statistically superior to oral semaglutide on HbA1c, and weight fell more with orforglipron than with oral semaglutide 14 mg.¹²⁴
• Gastrointestinal side effects, stopping treatment because of side effects and increases in pulse rate were all more common with orforglipron.¹²

Two details help when reading these numbers. The orforglipron trial doses do not match the marketed tablets: the 36 mg trial dose corresponds to the 17.2 mg Foundayo tablet.⁴ And the semaglutide doses, 7 mg and 14 mg, are the older strengths, equivalent to the 4 mg and 9 mg tablets now used in the UK.¹ A larger HbA1c fall in one trial does not settle which medicine suits a particular person; tolerability, licence and routine all matter.

Weight: ATTAIN and PIONEER are not comparable

Foundayo's weight results come from the ATTAIN trials:

• ATTAIN-1 (72 weeks, 3,127 adults with obesity and without diabetes): average weight fell 7.5%, 8.4% and 11.2% at the 6, 12 and 36 mg trial doses, vs 2.1% with placebo (a dummy pill). Between 5.3% and 10.3% of people on orforglipron stopped because of side effects, vs 2.7% on placebo.¹³
• ATTAIN-2 (72 weeks, 1,613 adults with obesity and type 2 diabetes): weight fell 5.1%, 7.0% and 9.6% at the three trial doses, vs 2.5% with placebo.¹⁴

Rybelsus is not licensed for weight loss, and its weight data come from diabetes trials. In the placebo-controlled PIONEER trials, average weight loss at 7 mg and 14 mg over 26 weeks ranged from about 2.3 to 4.4 kg, vs 0.4 to 1.4 kg with placebo.¹ These trials differ in length (26 vs 72 weeks), in who took part and in how weight is reported (kilograms vs percentage of body weight), so the figures cannot be ranked. Side effects and safety differences

For Rybelsus, the US label lists nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation as the most common side effects, mostly while the dose is being increased.¹ In ACHIEVE-3, gastrointestinal events were more frequent with orforglipron than with oral semaglutide.¹² Other differences:

• Contraception: Rybelsus did not change blood levels of the pill hormones ethinylestradiol and levonorgestrel.¹ The Foundayo label advises people on oral contraceptives to switch to a non-oral method, or add a barrier method such as condoms, for 30 days after starting and after each dose increase.⁴
• Thyroid warning: both US labels carry a boxed warning about thyroid C-cell tumours seen in rodents; the UK labels have no boxed warning.¹⁴²⁵
• Heart evidence: in SOUL (9,650 adults aged 50 or over with type 2 diabetes and cardiovascular disease, chronic kidney disease or both; average 47.5 months; vs placebo), major cardiovascular events occurred in 12.0% on oral semaglutide vs 13.8% on placebo.¹⁵ Foundayo has no cardiovascular outcome data yet.⁴¹²
• Pulse: the Rybelsus label notes small rises in heart rate of 1 to 3 beats per minute; in ACHIEVE-3, pulse increases were more common with orforglipron.¹¹²
