Rybelsus vs Wegovy: a diabetes tablet and a weight-management injection

Same active ingredient, different jobs. Rybelsus is a daily tablet for type 2 diabetes; Wegovy is a weekly injection for weight management.

Rybelsus and Wegovy both contain semaglutide from Novo Nordisk, but they are licensed for different things and taken in different ways. Rybelsus is a daily tablet licensed to lower blood sugar in adults with type 2 diabetes; it is not licensed for weight loss.12 Wegovy is a once-weekly injection licensed for weight management in adults and adolescents aged 12 and over, at a 2.4 mg weekly maintenance dose, with 7.2 mg also approved for some adults.4 In the US both also carry cardiovascular indications, but for different groups of people. Who each medicine is licensed for

  • Rybelsus: adults with type 2 diabetes, alongside diet and exercise, to improve blood sugar control. In the US it is also licensed to reduce the risk of major cardiovascular events (cardiovascular death, non-fatal heart attack or non-fatal stroke) in adults with type 2 diabetes at high risk of them. It is for adults only.12
  • Wegovy injection: weight management in adults and adolescents aged 12 and over. It also has an indication to reduce cardiovascular risk in adults with established cardiovascular disease and obesity or overweight in the US and, since 23 July 2024, in the UK; in the EU the supporting results are in the product information only.465 In the US and UK it is also approved for MASH (metabolic dysfunction-associated steatohepatitis, a form of fatty liver disease) with moderate to advanced fibrosis (scarring); this is not an EU indication.47

No regulator lists weight management as a Rybelsus indication. The weight loss seen in its diabetes trials is a secondary effect, and using Rybelsus purely for weight loss would be off-label, meaning outside its licence.132 Wegovy's licence, in turn, is for weight management rather than blood sugar control.4

Doses and routine

The doses cannot be compared in milligrams. Only a small share of a semaglutide tablet is absorbed (about 1% to 2% for the 1.5, 4 and 9 mg tablets and 0.4% to 1% for the 3, 7 and 14 mg tablets), so tablet doses are much larger in milligrams than injected doses.1

  • Rybelsus: 1.5 mg (or 3 mg) once a day for a month, then 4 mg (or 7 mg), with the option of 9 mg (or 14 mg) after at least another month. The maximum is 9 mg in the UK and 14 mg (or 9 mg as Ozempic tablets) in the US; the EU also allows 25 mg and 50 mg.213 Take it on an empty stomach with up to 120 ml of water, then wait at least 30 minutes before food, drink or other tablets.1
  • Wegovy: 0.25 mg once a week, increased over 16 weeks to the 2.4 mg maintenance dose. A 7.2 mg dose is also approved for some adults; the MHRA approved it in the UK on 6 January 2026.48

In practice that is a daily morning routine with fasting and a waiting time, against one injection a week. Some people prefer to avoid needles; others prefer not to plan their mornings around a tablet. They are not used together: Wegovy labelling says combining it with other semaglutide or GLP-1 medicines is not recommended.4 For the tablet routine, see how to take Rybelsus.

What the trials show

The two were tested in different people for different purposes, so their headline numbers measure different things.

Rybelsus was tested in the PIONEER trials in people with type 2 diabetes, with HbA1c (a blood test that reflects average blood sugar over recent months) as the main measure. In PIONEER 1 (26 weeks, against placebo, a dummy tablet), HbA1c fell 1.2 points with 7 mg and 1.4 with 14 mg, vs 0.3 with placebo, and weight fell 2.3 kg and 3.7 kg, vs 1.4 kg.1 Across the placebo-controlled PIONEER trials, weight loss at 7 mg and 14 mg over 26 weeks averaged about 2.3 to 4.4 kg, vs 0.4 to 1.4 kg with placebo.1

Wegovy was tested in the STEP trials for weight management. In STEP 1 (68 weeks, adults with overweight or obesity and without diabetes), average weight fell 14.9% with 2.4 mg vs 2.4% with placebo.10 In STEP UP (72 weeks, adults with obesity), it fell 18.7% with 7.2 mg, 15.6% with 2.4 mg and 3.9% with placebo.11

These trials differ in who took part (with or without diabetes), length (26 weeks vs 68 or 72 weeks), dose and how weight is reported (kilograms in PIONEER, percentage of body weight in STEP). Heart and liver indications

Each has its own cardiovascular outcome trial, in a different group of people:

  • SOUL (Rybelsus): 9,650 adults aged 50 or over with type 2 diabetes and cardiovascular disease, chronic kidney disease or both took oral semaglutide up to 14 mg or placebo for an average of 47.5 months. Major cardiovascular events occurred in 12.0% vs 13.8%, a 14% relative reduction (hazard ratio 0.86, where below 1 means fewer events on semaglutide).13 The FDA added a cardiovascular indication on 17 October 2025; in the EU and UK the results were added to the product information without a new indication.12
  • SELECT (Wegovy): tested the injection against placebo in adults with cardiovascular disease and overweight or obesity who did not have diabetes, and found fewer major cardiovascular events with semaglutide.12 The FDA approved a cardiovascular indication on 8 March 2024 and the MHRA on 23 July 2024; in the EU the results are in the product information only.965

Wegovy also has approvals for MASH with moderate to advanced fibrosis: US accelerated approval on 15 August 2025 and UK conditional approval on 3 July 2026. It is not an EU indication.47 Rybelsus has no liver indication.1

Side effects and safety

For Rybelsus, the US label lists nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation as the most common side effects. In placebo-controlled trials, 41% on 14 mg had a gastrointestinal side effect, vs 21% on placebo, and 8% stopped because of one, mostly while the dose was going up.1 Wegovy's label reports side effects from its own trials at its own doses, so the figures are not directly comparable.

Some safety points apply to semaglutide medicines more generally:

  • NAION, a sudden loss of vision in one eye: the EMA safety committee concluded in June 2025 that it is a very rare side effect of semaglutide medicines. Anyone with sudden vision loss should get urgent medical help.14
  • Pregnancy: the MHRA has reminded people not to use GLP-1 medicines in pregnancy or while breastfeeding, and to use effective contraception. Rybelsus should be stopped at least 2 months before a planned pregnancy.151
  • Surgery: there have been rare reports of stomach contents entering the lungs during general anaesthesia or deep sedation in people on GLP-1 medicines, so tell your healthcare team before any planned procedure.1
  • Other warnings in the Rybelsus label include pancreatitis, gallbladder problems, kidney injury from dehydration, and low blood sugar when combined with insulin or a sulfonylurea.1

Questions to discuss with a prescriber

This site cannot tell you which medicine to use. These are topics people often raise with a prescriber when both names come up:

  • What is the main goal? Rybelsus is licensed for blood sugar control in type 2 diabetes; Wegovy is licensed for weight management. Each licence reflects what the medicine was tested and approved for.14
  • Is there heart disease? The US cardiovascular indications cover different groups: Rybelsus for adults with type 2 diabetes at high risk, Wegovy for adults with established cardiovascular disease and obesity or overweight.14
  • Tablet or injection? A daily fasting routine against a weekly injection, and how other morning medicines would fit.
  • Age: Rybelsus is for adults only; Wegovy is licensed from age 12 for weight management.14
  • Current medicines: semaglutide products should not be combined, so any change from one to the other needs planning by your prescriber.4

Questions people ask

Is Rybelsus the same as Wegovy?

They contain the same active ingredient, semaglutide, from Novo Nordisk, but they are different products. Rybelsus is a daily tablet licensed for type 2 diabetes. Wegovy is a weekly injection licensed for weight management, with a 2.4 mg maintenance dose.

Can I take Rybelsus instead of Wegovy for weight loss?

Rybelsus is not licensed for weight loss by the FDA, the EMA or the MHRA. Weight loss in its trials was a secondary effect in people with type 2 diabetes, and using it purely for weight loss would be off-label. The semaglutide products licensed for weight management are Wegovy injections and the Wegovy pill. Your prescriber can explain which licensed options fit your situation.

Which is stronger, Rybelsus or Wegovy?

The doses cannot be compared milligram for milligram, because only a small fraction of a semaglutide tablet is absorbed, while an injection goes straight under the skin. The two were also tested in different people for different purposes: Rybelsus for blood sugar in type 2 diabetes, Wegovy for weight management. Neither set of trial results tells you how the other medicine would work for you.

Is there a tablet version of Wegovy?

Yes. The Wegovy pill is a once-daily semaglutide tablet licensed for weight management, with a 25 mg maintenance dose. It is a separate product from Rybelsus and is taken with the same empty-stomach routine. See our Rybelsus vs Wegovy pill guide.

Can you take Rybelsus and Wegovy together?

No. Both contain semaglutide, and Wegovy labelling says using it with other semaglutide products or GLP-1 medicines is not recommended. If you have been prescribed one and are offered the other, check with your prescriber or pharmacist.

Do Rybelsus and Wegovy lower the risk of heart attack?

Both have cardiovascular outcome trials, in different groups. SOUL tested Rybelsus in adults with type 2 diabetes at high cardiovascular risk; SELECT tested Wegovy injection in adults with cardiovascular disease and overweight or obesity without diabetes. In the US both carry a cardiovascular risk reduction indication for those groups. In the EU the results are in the product information rather than a separate indication, and in the UK only Wegovy has one.

Can I switch from Wegovy to Rybelsus?

They have different licences, so a change usually depends on why you are being treated, and only your prescriber can decide. Semaglutide stays in the body for weeks after the last dose, so do not stop one and start the other on your own.
