Rybelsus compared

Rybelsus vs Mounjaro: daily semaglutide tablet or weekly tirzepatide injection

Both treat type 2 diabetes, but one is a daily semaglutide tablet and the other a weekly tirzepatide injection that is also licensed for weight management in some countries. Here is what the labels and trials show.

Rybelsus and Mounjaro both treat type 2 diabetes, but they are different medicines taken in different ways. Rybelsus is a once-daily tablet of semaglutide, a GLP-1 receptor agonist (a medicine that copies the gut hormone GLP-1)1. Mounjaro is a once-weekly injection of tirzepatide, which acts on two hormone receptors, GIP and GLP-14. Mounjaro is also licensed for weight management in the UK and EU, and in the US the same medicine is sold for weight management as Zepbound; no regulator has licensed Rybelsus for weight loss6751. In trials, tirzepatide produced larger average falls in blood sugar and weight, but none of those figures comes from a trial that compared Mounjaro with Rybelsus directly121. Which medicine suits a particular person is a decision for their prescriber.

In the US, the newer 1.5, 4 and 9 mg Rybelsus tablets are now sold as Ozempic tablets; Rybelsus remains the name elsewhere10. See the Ozempic pill.

The trial rows come from different trials with different comparators, lengths and populations, so they cannot be compared with each other directly.

How they work: GLP-1 vs dual GIP/GLP-1

GLP-1 (glucagon-like peptide-1) is a hormone the gut releases after eating. Semaglutide copies it. It makes the pancreas release more insulin and less glucagon, but only when blood sugar is raised, and slightly slows how quickly the stomach empties1. It also reduces appetite, which is why weight often falls19.

Tirzepatide is a different molecule, not a version of semaglutide. It is a dual incretin medicine: it activates the receptors for two gut hormones, GLP-1 and GIP (glucose-dependent insulinotropic polypeptide)4. That puts Mounjaro in a different class from Rybelsus, Ozempic and Wegovy, which all contain semaglutide.

Both act on the GLP-1 receptor, and semaglutide labelling says using it with other GLP-1 receptor agonists is not recommended20. Any move from one medicine to the other is for your prescriber to plan.

Daily tablet or weekly injection

The biggest practical difference is the routine. Rybelsus is one tablet every morning on an empty stomach, swallowed whole with a sip of plain water (up to 120 ml, or 4 oz). You then wait at least 30 minutes before eating, drinking or taking other tablets, because food and drink reduce absorption1. The UK and EU labels also recommend fasting for at least 8 hours before the dose23. See how to take Rybelsus.

Rybelsus starts at a low dose for a month to reduce stomach side effects, then steps up; the starting dose does not control blood sugar on its own1. The top dose is 14 mg in the US (9 mg for Ozempic tablets), 9 mg in the UK and 50 mg in the EU, always as one tablet a day123. See Rybelsus dosage.

Mounjaro is injected once a week with no fasting rules. It starts at 2.5 mg and can rise in 2.5 mg steps, at least 4 weeks apart, to a maximum of 15 mg4. In the US it is also licensed for type 2 diabetes in children aged 10 and over; Rybelsus is for adults only41.

What each medicine is licensed for, by region

Rybelsus is licensed only for adults with type 2 diabetes, alongside diet and exercise12:

  • US: blood sugar control and, since 17 October 2025, reducing the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults at high risk of these events111.
  • EU and UK: type 2 diabetes that is not well controlled, on its own when metformin is not suitable or with other diabetes medicines. Heart trial results are in the product information, without a separate cardiovascular indication32.
  • Weight: no regulator lists weight management as a Rybelsus indication132.

Tirzepatide's licences depend on the brand name and the country:

  • US, Mounjaro: type 2 diabetes in adults and children aged 10 and over and, since 27 August 2026, cardiovascular risk reduction in adults with type 2 diabetes at high risk48.
  • US, Zepbound: weight management, and obstructive sleep apnoea (approved 20 December 2024)59.
  • UK and EU, Mounjaro: type 2 diabetes and weight management. The EU authorised Mounjaro on 15 September 2022; the MHRA authorised it for weight management on 8 November 202367.

Blood sugar and weight: what the trials show

Diabetes trials measure HbA1c, a blood test that reflects average blood sugar over recent months. The Rybelsus figures are the US label's treatment-policy results, which include people who stopped treatment1.

Rybelsus. In PIONEER 1 (26 weeks, 703 adults, compared with placebo), HbA1c fell by 1.2 points on 7 mg and 1.4 points on 14 mg, against 0.3 points on placebo1.

Mounjaro. In SURPASS-2 (40 weeks, adults with type 2 diabetes), tirzepatide lowered HbA1c by 2.01 to 2.30 points across its doses, against 1.86 points with the semaglutide 1 mg weekly injection (Ozempic)12.

SURPASS-2 is sometimes read as "Mounjaro vs Rybelsus", but its comparator was injected semaglutide, not the tablet. The UK SmPC says the 9 mg tablet gives exposure comparable to the 0.5 mg weekly injection, and that no tablet dose equivalent to the 1 mg injection has been established2. So SURPASS-2 cannot be turned into a tablet comparison. See Rybelsus vs Ozempic.

Weight. In the placebo-controlled PIONEER diabetes trials, average weight loss on 7 mg and 14 mg over 26 weeks ranged from about 2.3 to 4.4 kg, against 0.4 to 1.4 kg on placebo1. Tirzepatide's weight results come from separate weight-management trials: in SURMOUNT-1 (72 weeks, adults with obesity without diabetes, compared with placebo) weight fell by 20.9% on 15 mg against 3.1%13, and in SURMOUNT-5 (72 weeks, compared with the Wegovy 2.4 mg injection) by 20.2% against 13.7%14. Those trials differ in population and length, so they cannot be set against the PIONEER results. See Rybelsus and weight loss.

Heart outcomes and Mounjaro's 2026 US indication

A hazard ratio compares the rate of events between two groups; below 1 means fewer events on the trial medicine.

Rybelsus: SOUL. SOUL followed 9,650 adults aged 50 or over with type 2 diabetes and atherosclerotic cardiovascular disease (heart or circulation disease caused by narrowed arteries), chronic kidney disease or both, for an average of 47.5 months, comparing oral semaglutide (up to 14 mg) with placebo on top of standard care. A major cardiovascular event (cardiovascular death, non-fatal heart attack or non-fatal stroke) happened to 12.0% on semaglutide and 13.8% on placebo, a hazard ratio of 0.86, or 14% lower relative risk16. The FDA approved the US cardiovascular indication on the basis of SOUL on 17 October 2025111.

Mounjaro: SURPASS-CVOT. This trial compared tirzepatide with another diabetes medicine, dulaglutide, rather than with placebo. Major cardiovascular events happened to 12.2% on tirzepatide and 13.1% on dulaglutide (hazard ratio 0.92): non-inferior (not worse) but not superior15. On 27 August 2026 the FDA added a cardiovascular risk reduction indication to Mounjaro for adults with type 2 diabetes at high risk84.

So in the US both now carry a cardiovascular indication for adults with type 2 diabetes at high risk. One trial used placebo and the other an active medicine, so their percentages should not be lined up against each other.

Side effects and safety

Rybelsus. The most common side effects are nausea, abdominal pain, diarrhoea, decreased appetite, vomiting and constipation1. In placebo-controlled trials, nausea affected 20% on 14 mg against 6% on placebo, and 8% on 14 mg stopped because of stomach side effects against 1%, mostly while the dose was being increased1. Serious side effects include pancreatitis, gallbladder disease and kidney injury from dehydration after vomiting or diarrhoea1. See Rybelsus side effects.

Mounjaro. Its full side-effect list is in its patient leaflet and prescribing information4. Two differences from Rybelsus stand out:

  • Contraceptive pill: the MHRA advises people taking oral contraceptives to add a barrier method, such as condoms, for 4 weeks after starting tirzepatide and after each dose increase17. Semaglutide tablets did not change blood levels of the contraceptive hormones tested12.
  • Planning a pregnancy: the MHRA advises stopping tirzepatide at least 1 month before trying for a baby17; Rybelsus should be stopped at least 2 months before, because semaglutide takes weeks to leave the body12.

Shared warnings. Both US labels carry a boxed warning about thyroid C-cell tumours seen in rodents14; the UK and EU Rybelsus labels do not2. The MHRA's January 2025 advice on the risk of inhaling stomach contents during general anaesthesia or deep sedation covers both GLP-1 and dual GIP/GLP-1 medicines, so tell the team before any procedure18. Questions to discuss with your prescriber

  • Does a daily tablet with fasting rules or a weekly injection fit your routine?
  • What does the licence cover where you live, and is weight management part of the reason for treatment?
  • How do your heart and kidney history and other medicines fit? Insulin or a sulfonylurea raise the risk of low blood sugar with Rybelsus1.
  • What are your contraception and pregnancy plans, and what will it cost where you live (see the US and UK guides)?

Do not stop or change a diabetes medicine without speaking to your prescriber first.

Questions people ask

Is Mounjaro stronger than Rybelsus?

In its own trials, tirzepatide (Mounjaro) produced larger average falls in HbA1c and weight than Rybelsus did in the PIONEER trials. But those are separate trials with different comparators, lengths and populations. The main trial comparing tirzepatide with semaglutide in type 2 diabetes, SURPASS-2, used the semaglutide 1 mg injection, not Rybelsus tablets.

A larger average effect in a trial does not decide which medicine suits a particular person. Side effects, routine, other conditions and cost all matter, and that is a conversation for your prescriber.

Can you take Rybelsus and Mounjaro together?

They are not designed to be used together. Both act on the GLP-1 receptor, and semaglutide labelling says using it with other GLP-1 receptor agonists is not recommended. If you have been prescribed both, or are unsure whether a new prescription overlaps with one you already take, ask your prescriber or pharmacist before taking them.

Can I switch from Rybelsus to Mounjaro?

Moving between these medicines is a prescribing decision, because they are different drugs with different doses and routines. Do not stop Rybelsus or start Mounjaro on your own. If you have questions about your treatment, raise them with the prescriber who manages your diabetes.

Is there a Mounjaro pill?

No. Mounjaro (tirzepatide) is a once-weekly injection only. Eli Lilly's once-daily tablet is Foundayo, which contains a different medicine, orforglipron.

Rybelsus is a tablet, and in the US its newer 1.5, 4 and 9 mg tablets are now sold as Ozempic tablets.

Is Mounjaro the same as Rybelsus?

No. Rybelsus contains semaglutide, the same medicine as Ozempic and Wegovy, and works on the GLP-1 receptor. Mounjaro contains tirzepatide, a different molecule made by Eli Lilly that works on both the GIP and GLP-1 receptors. They also differ in form: Rybelsus is a daily tablet, Mounjaro a weekly injection.

Which causes more weight loss, Rybelsus or Mounjaro?

The two cannot be compared fairly from the published numbers. Rybelsus is not licensed for weight loss; weight change was a secondary effect in its diabetes trials, where average losses over 26 weeks were a few kilograms. Tirzepatide's weight figures come from separate weight-management trials such as SURMOUNT-1, run for 72 weeks in people without diabetes and reported in percentages.

Mounjaro is licensed for weight management in the UK and EU, and tirzepatide is sold for weight management as Zepbound in the US. Whether a weight-management medicine is appropriate for you is for your prescriber to assess.

Does Mounjaro affect the contraceptive pill?

The MHRA advises people taking oral contraceptives to add a barrier method, such as condoms, for 4 weeks after starting tirzepatide and for 4 weeks after each dose increase. Semaglutide tablets did not change blood levels of the contraceptive hormones tested in studies. If you use hormonal contraception, check with your prescriber or pharmacist.
